fix(setup): handle existing MCP config by offering to replace instead of failing ... 23076a9a81

When `claude mcp add` fails with "already exists", the wizard now asks
to replace the existing config instead of printing manual JSON instructions.
On confirmation, removes the old config from all scopes and retries.
Fallback messages now show the actual `claude mcp add` command rather
than raw JSON config blocks.
